To strengthen the campus epidemic prevention and to cooperate with the guidelines of the Ministry of Education and the Central Epidemic Command Center, CMU students can apply for Vaccination Leave and Epidemic Leave from now on. Both leaves will not affect your absence record and the regulation is as follows:
Vaccination Leave
- Students that are vaccinated (including those with adverse reactions) can apply for Vaccination Leave up to four days after vaccination.
- Please apply for Vaccination Leave with your Student Leave System. You may apply for Vaccination Leave afterwards, but please inform your course instructor so that the instructor can prepare for the online course.
- To apply for Vaccination Leave, please submit your “COVID-19 Vaccination Record Card” with the vaccination date. No other documentation is required. (For example: If your vaccination date is Sep 22, you may apply for Vaccination Leave from Sep 22-25.)
- Vaccination Leave will not affect your absence record. If you are in good health condition, please participate in the online course.
- If you feel unwell for more than three days after vaccination and need to seek for medical treatment, you may apply for sick leave.
Other notices:
- Vaccination Leave does not necessarily need to be applied on the day of vaccination. Instead, it should be applied when you feel unwell after vaccination. The application of Vaccination Leave should be a one-time event.
- For teachers: If you receive students’ application for Vaccination Leave, please do care about their health condition and assist them to take the online course.
Epidemic Leave
- If you need to go through home isolation, home quarantine, or self-health management, please submit the official document (for example: cell broadcast message) from the official health authority or the government announcement.
- If you have COVID-19 symptoms and the doctor recommend you to start a self-health management, please submit medical certificate to the Health Center to apply for Epidemic Leave.
- Students who go abroad without the approval of the university and need to go through home isolation, home quarantine, or self-health management after returning to Taiwan are not allowed to apply for the Epidemic Leave. The leave will be counted as sick leave or personal leave.

New！COVID-19「疫苗接種假」及「防疫假」注意事項！
本校為強化校園防疫，並配合教育部與中央流行疫情指揮中心相關指引，增列「疫苗接種假」及「防疫假」，以上假別不列計同學出缺席紀錄；相關檢附資料如下：
疫苗接種假
- 本校學生前往接種疫苗（含接種後發生不良反應），得自接種之日起四日內可申請疫苗接種假三天。
- 如同學需請疫苗接種假，請至本校「學生請假系統」提出申請。「疫苗接種假」可事後請假，但請先向授課老師說明，以利老師準備同步或非同步線上課程。
- 申請疫苗接種假，須檢具疫苗接種紀錄卡(黃卡)佐證上傳請假系統，無需就診或其他證明（需填寫接種日期，於接種日起四日內可請疫苗接種假三天，例如9/22施打，9/22-9/25其中三日可請疫苗接種假）。
- 疫苗接種假由授課教師比照生理假核准，疫苗接種假不列入缺課紀錄，若身體狀況良好仍請參與線上課程。
- 如接種疫苗超過三日仍有不適症狀需就醫及休養，可請一般病假。
補充說明：
- 上述例子並非指學生一定需申請疫苗注射日當天，而應該是以因接種疫苗後產生身體不適才申請，學生需要於事件發生後一次申請。
- 如接獲學生申請疫苗接種假，煩請老師們關心學生身體狀況，並協助學生採同步或非同步線上授課方式學習。
防疫假
- 經衛生主管機關匡列為居家隔離、居家檢疫或自主健康管理者，須檢附衛生主管機關開立檢疫、隔離或政府公告或足以證明之文件與資訊(如細胞簡訊)。
- 若有COVID-19相關症狀，醫囑建議返家自主管理須請假者，請上傳「就醫相關證明」，後續由健康中心檢視相關證明方予給假。
- 疫情期間，學生非經校方核准出國，返國後須自主健康管理、居家檢疫或居家隔離者，不得申請防疫假，應列入當學期事病假日數。
