防疫門禁措施調整(CMU campus access control )
•校外訪客
1.入校當日進入COVID-19防疫專區的「校外訪客健康聲明憑證」填寫健康聲明。
2.填寫完成會收到當日有效之「個人健康聲明憑證」，可先截圖保存，以免遺失。
3.入校時請出示當日有效「個人健康聲明憑證」供各出入口防疫站檢測人員核對後，即可快速通關。亦可於有刷卡機之入口使用身分證刷卡入校。
•校內教職員生(含校內人士)
1.維持現行規定，刷卡入校。
2.汽機車道：依車牌辨識實聯制入校，車主請出示學生證或教職員工證入校。
•For visitors
1.On the day of entry, please go to the “COVID-19 pandemic prevention area” of CMU website and fill out the “Visitor’s Health Declaration Form”.
2.After completing the form, you will receive a valid “Health Declaration Form” which is a one-day pass. You can save the screenshot for a fast pass on that day.
3.Please show the valid “Health Declaration Form” to the gate security guard, you will be allowed to enter the campus once they have checked your application. An alternative way to enter the campus is to scan you ID at any gate that is equipped with the scanner.
•For CMU members
1.Please scan you ID to enter the campus.
2.For car drivers and motorcyclists：Please show your CMU ID or student ID.
