110學年度第二學期即將開學，同學將陸續返校上課，為確保同學們入校健康安全，以免造成防疫破口之風險，本校經第150次「嚴重特殊傳染性肺炎」防疫小組應變會議決議，將透過健康聲明，查核健康狀況及確認接觸史，請各位同學配合以下相關防疫措施：
1.請全校同學於返校前務必至校園首頁：『防疫專區/校內人員健康聲明入口』 填寫健康聲明(https://2019ncov.cmu.edu.tw/)，自2月15日起，未完成填答，刷卡入校時將顯示藍色訊息，無法進入校園！
2.由車道入校同學(乘客仍需下車刷卡入校)，2月底前可出示APP中”學生健康聲明完成憑證“，免刷卡。量測體溫後，可快速通關入校。
3.入校採實聯制、實施體溫監測，於校園內須全程配戴口罩。

Dear CMU students,
The new semester is about to start. To ensure the health and safety of all CMU members, please do comply with the following pandemic prevention measures suggested by the 150th meeting of the CMU Epidemic Prevention Team:
1.Before returning to the campus, please fill out the Survey for Severe Special Infectious Pneumonia Health Declaration (防疫專區/校內人員健康聲明入口 https://2019ncov.cmu.edu.tw/). From February 15, all students must complete the Health Declaration Form prior to entering CMU campus with student ID cards. You will not be able to enter the campus if you did not complete the declaration form.
2.Please show the valid “Health Declaration Form” to the gate security guard, you will be allowed to enter the campus once they have checked your application. An alternative way to enter the campus is to scan you ID at any gate that is equipped with the scanner.
3.Whenever entering the campus, please register in the real-name registration system and cooperate with the guards to take body temperature. Do wear a mask all the time while staying on campus.
