感謝您日前配合本校完成第一階段的中港澳旅遊史問卷調查，接下來為了開學後校園您我的健康，我們將再進行一次中港澳日韓旅遊史(含轉機)及未來規劃出國時間及地點的調查問卷，請大家務必配合於109年2月29日前至校園首頁防疫專區/校內人員通報入口(https://2019ncov.cmu.edu.tw/)填妥完畢，各項資料如未完整提供完畢，將可能觸及政府公告之第五類法定傳染病相關各項防疫措施，違及校內師生健康利益。敬請各位協助與配合！
由中港澳韓入境之學生，請於「入境後起14日內」在家進行居家檢疫，居家檢疫期間不能外出，也不得出境或出國及搭乘大眾運輸工具，如違反規定，依「傳染病防治法」第69條規定可處1萬至15萬元不等罰鍰。

For the campus and your own safety, please complete the survey for travel history of China, Hong Kong, Macau, Japan, and Korea (including transfer), and future travel plan (including date and destination). Please complete this survey before February 29, 2020(校內人員通報入口: https://2019ncov.cmu.edu.tw/). Failing to provide accurate information on time may violate the regulation of the epidemic preventive measures announced by the government and do harm to the health of the campus. For students entering Taiwan from China, Hong Kong, Macau, Japan, and Korea, a 14-day home quarantine is required. During home quarantine, you are not allowed to leave home, go abroad, or take public transportation. If any violation is made, you may be fined NT$10,000 -150,000 according to the Infectious Disease Control Act.
